About
Our Mini-Grants
Woodburn
Together is interested in funding proposals from Woodburn
community groups and Woodburn School District staff that
address one or more community risk factors and contribute
to one or more Assets in children and youth, from the Search
Institute 40 Developmental Assets. Please click here for
What is Healthy Communities, Healthy Youth?, with
a description of the 40 Developmental Assets. The application
is downloadable below.
Please
use the second page of the form below to report the results
of your program to Woodburn Together. The evaluation report
is due within 30 days of completion of the program. Failure
to submit an evaluation report jeopardizes our coalitions
ability to secure future funding and your organization will
not qualify for any future grants from Woodburn Together.
